OCERS offers a full compliment of online and printable forms for its members and employers to assist with the various changes in one’s career and life.

When possible we recommend that our members login to the myOCERS portal to submit requests online. When that is not possible, please print, sign and return the completed form(s) to OCERS.

Some forms may contain sensitive information and should never be sent via Email, Fax or Instant Messaging without proper security and/or encryption protection. If you cannot locate a specific document or encounter technical difficulties when downloading a form, please contact us.


Member Forms

Active members can login to their myOCERS account to update beneficiary information and request to purchase service credit; retirees can submit address changes, direct deposit updates, tax withholding elections as well as update their beneficiary information.
